Trellis RFI Forms

You can now natively embed Trellis RFI (Request for Information) / subscription forms in Quickstart.

Trellis Form is an Experimental Module

The functionality of this module may change over time as it is improved. Currently, the module is intended to be used with RFI / subscription forms only. Please let your Trellis contact know that you intend to publish this form on a Quickstart site so that they can provide you with a properly formatted Trellis form URL.

The Quickstart Media - Trellis Form module is not enabled in Quickstart by default. To enable the module, an Admin needs to go to Manage Extend and search for Quickstart Media - Trellis Form and install it.

How to add a Trellis form

Navigate to Manage > Content > Media and press Add media. Select Trellis Form and enter a Trellis Form URL and a descriptive name. The descriptive name is used in the editing interface to help manage and organize multiple forms. It is not exposed to the public.

How to embed a Trellis form

Since Trellis forms are treated as media in Quickstart, they can be embedded in most text areas using the Insert Media button. After you have added your Trellis form to your Quickstart site, locate a text area where you want your Trellis form and use the Insert Media button to select and insert your Trellis Form.

Contact the Trellis team at https://trellis.arizona.edu to request a URL to your form that works with Quickstart.

The Trellis form URL provided by the Trellis team is specifically formatted to support dynamic pre-filling of certain form fields. At this time, the Quickstart Media - Trellis Form module supports rendering RFI / subscription forms in the following format https://forms-a.trellis.arizona.edu/<FORM_ID>?tfa_4=<CAMPAIGN_ID>&tfa_7=<CAMPAIGN_NAME>&tfa_9=<CAMPAIGN_DESC> where:

  • <FORM_ID> is the identifier for your form type. Do not change this value unless instructed to by your Trellis support contact.
  • <CAMPAIGN_ID> corresponds to your specific campaign of this form. Do not change this value unless instructed to by your Trellis support contact.
  • <CAMPAIGN_NAME> is the user-friendly title of your campaign. You may change this value to adjust what the user sees when the form is rendered.
  • <CAMPAIGN_DESC> is the user-friendly description of your campaign. You may change this value to adjust what the user sees when the form is rendered.
